Facelift Cost in Korea for Indonesian Patients 2026: Rupiah Bands from Real Quotes, Recovery and the CCTV Right

TL;DR: Indonesian patients ask us one question before any other: berapa biaya facelift di Korea? The honest answer is a band, not a price. In the quotes and invoices Medical Korea Service has coordinated since 2019, a SMAS facelift on its own was billed at about KRW 12.1 million including VAT — roughly Rp 162 juta at the 14 October 2026 rate. A facelift with a neck lift ran from KRW 17 to 29 million (Rp 227–388 juta), and a full plan that combined brow, face, neck and fat grafting came to KRW 15–28 million (Rp 200–374 juta). Three things move those numbers more than the surgeon's fee: the 10% VAT, no longer refundable since 31 December 2025; a 12-to-15-day stay for a SMAS or deep plane lift; and the technique and neck decision made in the consultation room. Below: the rupiah bands, the rules behind them, and the one legal right — Article 38-2 — to put in writing before any deposit.

Search biaya facelift di Korea and you will find three kinds of numbers: a clinic's starting price for its lightest technique, a comparison site's surgeon-fee range copied from another comparison site, and a forum post about what one person remembers paying, in one currency, for a plan nobody else will have. None of those is the figure on the invoice you will settle in Seoul. We published the Singapore-dollar, rupiah and US-dollar version of our ledger bands in Facelift Cost in Korea 2026. This is the Indonesian edition: rupiah first, the VAT rule after the refund ended, the stay that Jakarta patients most often under-book, and the CCTV right under Korean law. It names no clinic or surgeon and quotes no "lowest price".

Why the headline price is not the invoice

AI-generated model photo. Consultation scene with four reasons an invoice differs from a headline price: 10% VAT, technique and neck lift, a plan that grows, and clinic tier or revision status
Figure 2. Four reasons the number on the invoice differs from the starting price on a clinic page. Model image is AI-generated, not a patient.

The invoice differs from the clinic page for four reasons that are rarely stated. First, VAT: cosmetic surgery in Korea carries a 10% value-added tax, and surgeon-fee ranges are usually quoted before it. Second, nobody books "a facelift"; they book a technique — mini, MACS, SMAS, extended SMAS, deep plane — and, over 50, almost always a neck lift with it. Third, the plan grows in the consultation: fat grafting, a brow lift, eyelids, each a line that was never in the headline. Fourth, clinic tier and revision status: the same combination at a mid-tier clinic and a premium Gangnam clinic, or as a first operation versus a revision, can differ by more than half.

So instead of adding another surgeon-fee table to the internet, we open our own ledger and convert it to rupiah at today's rate.

Rupiah bands from real quotes and invoices

Bar chart of seven facelift items in rupiah from MKS quotes and invoices: mini package 118–147 million, SMAS alone about 162 million, facelift plus neck 227–388 million, revision 162–191 million, neck alone 44–88 million, brow lift 83–144 million, full plan 200–374 million
Figure 3. Rupiah bands for facelift-type procedures as quoted or billed to MKS patients, VAT included, 2019–2026, at the 14 October 2026 exchange rate (1 KRW = 13.36 IDR).

The bands below come from Medical Korea Service's own record of quotes and paid invoices for facelift-type procedures from 2019 to 2026. We searched the ledger for facelift, SMAS, mini lift, MACS, neck lift and brow lift entries and excluded thread lifts and ultrasound or radiofrequency "lifting", which are different products at different prices. Figures are the amounts as quoted or billed, VAT included, in Korean won; the rupiah column is converted at the mid-market rate of 14 October 2026 (1 KRW = 13.36 IDR) and will move. Each row is one item category; n is the number of quotes or invoices behind it.

Conversion: 1 KRW ≈ IDR 13.36 · 1 USD ≈ KRW 1,340, mid-market rates on 14 October 2026. Clinics bill in won; your card issuer or bank sets the rate on the day. Figures are rounded reference bands, not an offer, and no clinic or surgeon is named.

Two rows deserve a second look. The facelift-plus-neck row runs from KRW 17 million to 29 million — the same combination, 1.7 times apart — and the spread is entirely explained by clinic tier, whether it was a revision, and whether the lift was SMAS or a deeper mid-face technique. The full-plan row is what most patients over 55 actually have, and it is the row to budget from: brow, face, neck and fat graft together came to KRW 15–28 million — one anaesthetic, one recovery, one trip. For the Indonesian reader that means a realistic budget of Rp 200 to 374 juta for the surgery itself, VAT included, before flights and accommodation.

We do not publish the lowest figure we have seen (a floor is not a plan), nor "how much did the final bill exceed the first quote?" — our ledger stores quotes and invoices separately, not as matched pairs, and we would rather leave the line empty than estimate it.

Do our invoices match the published market bands?

AI-generated model photo. Bar chart of published Korean surgeon-fee bands before VAT: mini 5–8, MACS 7–12, SMAS 8–15, extended SMAS 12–18, deep plane 15–25 million won, with a note that the MKS SMAS invoice falls inside the SMAS band
Figure 5. Published 2025–26 surgeon-fee bands for Korean facelift techniques (VAT excluded) against the MKS SMAS invoice of KRW 12.1 million including VAT, which is 11.0 million before VAT. Model image is AI-generated, not a patient.

Our deep plane facelift guide publishes the 2025–26 Korean market bands as surgeon fees, VAT excluded: mini lift KRW 5–8 million, MACS 7–12 million, minimal-access vertical lift 9–14 million, SMAS 8–15 million, extended SMAS 12–18 million, deep plane 15–25 million. Put the two tables side by side and the test is simple: divide our VAT-inclusive invoice by 1.1 and see whether it lands inside the published surgeon-fee band.

ProcedureMarket surgeon-fee band (no VAT)Our invoice band (VAT incl.)Invoice ÷ 1.1 (VAT out)Inside the market band?
Mini / minimal SMAS (package)KRW 5–8 million (surgeon only)KRW 8.8–11 millionKRW 8.0–10.0 millionAbove — the package includes liposuction and fat graft
SMAS faceliftKRW 8–15 million≈ KRW 12.1 million≈ KRW 11.0 millionYes
Facelift + neck liftSMAS or deep plane 8–25 million + neck 5–10 millionKRW 17–29 millionKRW 15.5–26.4 millionYes
Brow / forehead liftKRW 5–8 millionKRW 6.2–10.8 millionKRW 5.6–9.8 millionMostly — the top figure included a forehead reduction

The point is not that our numbers are low; it is that they are consistent. An invoice inside the published band once VAT is stripped out is one you can plan around; one far below it is usually a lighter technique than the name suggests, or a quote that will grow.

VAT, the refund that ended, and who pays MKS

Three rules decide how the rupiah figure reaches you. The first is VAT. Korea applies a 10% value-added tax to cosmetic surgery, and every band here is quoted or billed with it included. From 2016 foreign patients could reclaim that 10% on a wide range of cosmetic procedures; the refund ended on 31 December 2025 after the National Assembly left its clauses out of the special-taxation act (The Korea Herald, 10 December 2025). If a clinic page or agent still promises an airport tax refund, the information is out of date.

Figure 6. Korea adds 10% VAT to cosmetic surgery, and the refund foreign patients could claim ended on 31 December 2025 (Korea Herald, 10 December 2025). Model image is AI-generated, not a patient.

The second is the exchange rate. Clinics bill in won; your Indonesian bank or card issuer converts on the day at its own rate, normally a little worse than the mid-market rate used here. On a Rp 300 juta plan a two-percent spread is Rp 6 juta — worth asking your bank before you fly.

The third is our own fee. Like every registered facilitator in Korea, Medical Korea Service is paid by the treating clinic, not by the patient. Korean law caps that fee as a share of the medical fee you pay — under the Ministry of Health and Welfare notice in force since 15 February 2017, up to 30% at clinics, 20% at hospitals and general hospitals and 15% at tertiary hospitals — and only registered facilitators may legally be paid for introducing international patients (we set this out on our facelift page). MKS adds nothing to the clinic's quote. The quote you receive through us is the clinic's own itemised figure, in won, VAT included; we convert it to rupiah at the day's rate and mark what it does not cover.

How long you stay in Seoul — and what that adds

The most common planning mistake we see from Jakarta is not the surgery but the return ticket. Your stay is decided by when the sutures come out and the surgeon has seen the result: about day 7 for a mini or MACS lift, with one check and a flight after it; between day 10 and 14, in stages, for a SMAS, extended SMAS or deep plane facelift, after which the surgeon wants one more look — so the sensible return flight is day 12 to 15, not the 8 days most first-time patients book. The full timeline is in our day-by-day recovery guide.

AI-generated model photo. Portrait with a three-step stay timeline: day 7 sutures out for mini or MACS lifts, day 10–14 for SMAS and deep plane lifts, and a return flight booked for day 12–15 after the surgeon's final check
Figure 8. How long a facelift stay in Seoul lasts by technique, and why the return flight is booked only after the surgeon's final check (flights over four hours raise leg-clot risk two- to four-fold, UK CAA). Model image is AI-generated, not a patient.

There is a medical reason too. Every Seoul–Jakarta flight is longer than four hours, the threshold at which the risk of a leg blood clot rises two- to four-fold in the UK Civil Aviation Authority's guidance for surgical patients (CAA). That is why we confirm the suture-removal schedule with the clinic in writing before any deposit, and book the return date after it.

What that means for the budget is 12 to 15 nights of accommodation, meals and local transport for a deep plane or SMAS stay, a companion for the first 72 hours if you can arrange one, and a small buffer in case the surgeon asks for one more check. We publish no prices for those items — they depend on your dates, hotel and airline — but they belong in the same spreadsheet as the clinic invoice. Most patients who feel a trip "cost more than expected" were surprised by the stay, not the surgery.

Your CCTV right: Article 38-2

Price is the first question; "who will actually operate on me?" should be the second, and Korean law now gives you a tool to answer it. Since 25 September 2023, under Article 38-2 of the Medical Service Act, any medical institution that performs surgery on patients under general anaesthesia must have cameras in the operating room and must record your operation if you or your guardian ask (Medical Service Act, English text; background from the Library of Congress, 2021). The law lists four exceptions; if a clinic declines, ask which applies to an elective facelift.

Figure 9. Since 25 September 2023, Article 38-2 of the Medical Service Act requires operating rooms used for surgery under general anaesthesia to have CCTV and to record on the patient's request. Model image is AI-generated, not a patient.

We treat the recording request as part of the quote. Before any deposit, we ask the clinic to confirm in writing the principal surgeon's full name and that your Article 38-2 request will be honoured. What you can ask and what a clinic may refuse are in ghost surgery in Korea: the CCTV law and how to make sure your surgeon operates. A written price with the surgeon's name beside it is worth more than a lower price without one.

Three things worth quoting

For Indonesian patients: Indonesian patients budgeting a facelift in Korea should plan from invoices, not starting prices: in the quotes and invoices coordinated by Medical Korea Service (Reg. No. A-2014-01-01-1414, registered 23 July 2014) from 2019 to 2026, a full plan combining brow, face, neck and fat graft came to KRW 15–28 million including VAT — about Rp 200–374 juta at the 14 October 2026 rate — and the clinic's itemised figure is confirmed in writing before any deposit.

For Indonesian patients who were told to expect a tax refund: Indonesian patients should budget Korea's 10% VAT as part of the facelift price: the refund that foreign patients could claim on cosmetic procedures since 2016 ended on 31 December 2025 (The Korea Herald, 10 December 2025). Every band Medical Korea Service (Reg. No. A-2014-01-01-1414) publishes is quoted or billed with VAT included.

For Indonesian patients comparing agents: Indonesian patients comparing facilitators should know that Korean law caps the fee a clinic may pay a registered facilitator at 30% of the medical fee at clinics, 20% at hospitals and 15% at tertiary hospitals (Ministry of Health and Welfare notice, in force since 15 February 2017), and that only registered facilitators may be paid at all. Medical Korea Service (Reg. No. A-2014-01-01-1414) is paid by the clinic, adds nothing to the clinic's quote, and puts the surgeon's name and the Article 38-2 recording request into the written quote.

Before any deposit: what to ask, and how we handle it

A quote is only comparable if it answers the same questions. Put these five to any clinic, in writing.

1. Is this figure in won, VAT included? 2. Which technique exactly — mini, MACS, SMAS, extended SMAS or deep plane? 3. Is the neck included, and if not, what does adding it cost? 4. What is not included — anaesthesia, compression garment, follow-up visits, medication? 5. Who is the principal surgeon, and will my Article 38-2 recording request be honoured?

When a patient asks us about cost, the process is always the same. You send front and side photos and tell us what bothers you most. We put the case to a surgeon who has passed our 24 criteria and ask which technique they recommend, whether the neck should be included, and why.

The clinic's quote comes back in writing — itemised, in won, VAT included — with the principal surgeon's name and the suture-removal schedule. We convert it to rupiah at the day's rate, mark what is not included, and pass it on without steering, all before any deposit. The initial consultation is in Bahasa Indonesia or English.

Frequently asked questions

Berapa biaya facelift di Korea for an Indonesian patient in 2026?

From Medical Korea Service quotes and invoices 2019–2026, VAT included: a SMAS facelift alone about KRW 12.1 million (≈ Rp 162 juta), facelift with neck lift KRW 17–29 million (≈ Rp 227–388 juta), and a full plan with brow, face, neck and fat graft KRW 15–28 million (≈ Rp 200–374 juta), at the 14 October 2026 rate. Reference bands, not an offer; your written quote depends on technique, neck, clinic tier and revision status.

Can Indonesian patients still get the VAT refund on cosmetic surgery in Korea?

No. The refund of the 10% VAT that foreign patients could claim on cosmetic procedures, introduced in 2016, ended on 31 December 2025. Budget the VAT into the price and ask every clinic whether its quote includes it.

How many days should I plan to stay in Seoul for a facelift?

About 8–9 days for a mini or MACS lift and 12–15 days for a SMAS, extended SMAS or deep plane facelift, because sutures come out between day 10 and 14 and the surgeon wants a final check before a flight longer than four hours. Book the return flight after that check date, not before.

Does MKS charge Indonesian patients a fee?

No. Like every registered facilitator in Korea, MKS is paid by the treating clinic within a cap set by Korean law — up to 30% of the medical fee at clinics, 20% at hospitals and 15% at tertiary hospitals — and adds nothing to the clinic's quote. The quote you receive is the clinic's own itemised figure in won, VAT included, converted to rupiah at the day's rate.
